MINERAL RIDGE - John Henry Walchak, 65, passed away Friday, Nov. 16, 2012, at University Medical Center in Pittsburgh from leukemia.
John was born Dec. 10, 1946, in Warren, a son of Henry and Grace Fillmore Walchak.
Survivors include his wife Carla; a daughter, Kelli Cooper; two brothers; and a grandson.

Friends may call from 5 to 7 p.m. Tuesday at the Lane Family Funeral Homes Roberts-Clark Chapel. Services will be at 11 a.m. Wednesday at the Central Christian Church in Warren. Burial will be at Pineview Memorial Park.
