WARREN - SCOPE Inc. of Trumbull County's certification as a long-term care agency has been revoked, according to a decision from the director of the Ohio Department of Aging.
Bonnie Kantor-Burman's determination released Tuesday also says that any provider whose certification has been terminated is prohibited from reapplying for certification.
The order is effective immediately.
The sanction is a result of SCOPE failing to meet a number of state requirements for employee criminal background checks and credentials.
